Sun Damage

Photodynamic therapy (PDT) is a two-stage treatment uses light energy and a cream to destroy cancerous and precancerous cells.

These treatments combined remove sun damaged precancerous spots, and also improve sun-damaged skin, fine lines, blotchy pigmentation, and rough skin patches. PDT also has the unique ability to minimize pores and reduce oil glands-effectively treating stubborn acne vulgaris, acne, Rosacea, and improving the appearance of some acne scars.

What are Actinic Keratoses (AK)?

Actinic keratoses (AK) are often called “sun spots”. They are rough textured, dry scaly patches on the skin that are caused by excessive exposure to ultraviolet light (UV) such as sunlight. Up to 58 million Americans have AKs.

Actinic keratoses are the most common precancer and are considered to be the first step in the development of skin cancer. They have the potential to progress to squamous cell carcinoma (SCC). While most AKs remain benign, a study has shown that approximately 10 percent develop into SCC within an average of 2 years.

A medication called Levulan is applied to the area of sun damage that has been evaluated and diagnosed by one of our dermatologic health care providers. The medication is absorbed by the damaged cells after a prescribed “incubation” period. The area is then exposed to the LED light for a specific amount of time which causes a chemical reaction destroying only the damaged cells. After the damaged cells are destroyed, healthy new cells will replace them.

After photodynamic sun avoidance for 24-48 hours is necessary. Cleanse treatment area with a gentle cleanser such as Cetaphil. A sunscreen with an SPF of 30 or above is necessary, even if you are not planning on going outside. We suggest a sunscreen that contains zinc or titanium. Appropriate sunscreens are available for purchase in our office. If your skin becomes very irritated, use an over the counter 1% Hydrocortisone Cream twice a day.

Overall, patients with severely sun-damaged skin with actinic keratosis, mottled pigmentation, dull or sallow skin, and skin laxity, may see good to excellent improvement with PDT. There can be improvement of large pores, non-pitted acne scars, and active acne. Depending on the area being treated and the recommended incubation time, different numbers of treatment sessions spaced 4 to 6 weeks apart may be required to achieve the desired improvement and reduction in lesions. On average, a series of two to three treatments are performed. It is not always possible to predict ahead of time how many treatments your specific condition may take or how you will respond to PDT.

How is Photo Rejuvenation performed?

A photo rejuvenation procedure can take anywhere from 15 to 45 minutes, depending on the size of the area to be treated. During the procedure, pulses of intense light are delivered to the skin through very specific filters. The level of energy is delivered in specific settings to maximize the results. The tissues absorb the light energy which stimulates the production of collagen. With new collagen forming, your aging skin begins to soften. Fine lines and wrinkles are decreased and the redness with rosacea can be dramatically reduced. This procedure is repeated in a series of six treatments, every three to four weeks, in order to achieve optimal results. The number of treatments can vary depending on the area and the skin’s condition. Even after one treatment, most patients notice a smoother look and feel to their skin. Improvements will become more evident with every treatment.

Most patients describe the treatment like that of a rubber band being snapped on their skin. For those people sensitive to pain, a topical anesthetic may be applied.

Post-operative discomfort from photo rejuvenation is minimal to non-existent. Bruising and redness may occur, but this is temporary.

Photo rejuvenations can have a lasting effect on the patient. However, this treatment does not prevent the skin from aging. Patients will still continue to age, and a follow up treatment is usually done after a year or so. Otherwise, if the patient does not undergo further treatment, their skin will revert to a pre-treatment appearance. For those patients undergoing photo rejuvenation to treat rosacea, the redness and flushing can be reduced by 80 to 90 percent.

What is Melasma?

Melasma is a skin condition characterized by brown patches on the skin often common among women. These tan light patches or spots appear symmetrically on the cheeks and around the eyes. Everyone’s skin responds a bit differently, and your results will depend on your compliance with your clinician’s instructions, however, most patients begin to see results between their second and third treatments.

To begin, a topical anesthetic cream is applied to the treatment area roughly 30 – 60 minutes in advance of the procedure. The device’s tiny insulated needles are then used to penetrate deep into the dermis and effectively heat the tissue to the desired temperature within microseconds. Not including the time required for the local anesthetic cream to take effect, the entire process takes approximately 30 – 60 minutes, depending upon the area being treated. Afterwards the body’s natural healing process immediately kicks into gear to produce a new, healthier, tighter skin structure.
